As a new type of lipid-lowering drug, the contraindications, food restrictions, and medication conditions of Nilemdo need to be strictly controlled. 1. Taboo population
(1) Pregnant women
Based on animal studies showing the risk of fetal toxicity, bevacillin is contraindicated during pregnancy, and pregnancy status must be confirmed and effective contraceptive measures taken before use.
(2) Lactating women
Medications may be secreted through breast milk. Breastfeeding should be stopped during the treatment period and at least 3 days after the last administration to avoid potential harm to the infant.
(3) Severe liver dysfunction
Child Pugh Class C patients are contraindicated as the drug is mainly metabolized by the liver, which may exacerbate liver function damage and increase the risk of adverse reactions.
2. Forbidden foods
(1) Grapefruit and its products
Containing CYP3A4 potent inhibitor ingredients, it can significantly increase the blood concentration of beperidic acid and increase the risk of adverse reactions such as rhabdomyolysis.
(2) High-fat diet
It is recommended to avoid taking it with high-fat meals and administer it on an empty stomach or after a low-fat diet to prevent affecting the drug absorption rate and bioavailability.
3. Restrictive conditions
(1) Liver function monitoring
ALT/AST should be tested before treatment and 3 months after medication. If it continues to exceed 3 times the upper limit of normal values, the medication should be stopped and the degree of liver injury should be evaluated.
(2) Drug interactions
Prohibiting the combination use of OATP1B1 inhibitors such as cyclosporine may increase the exposure of beperidic acid by 16 times, leading to serious adverse reactions.
(3) Adjustment for special populations
Patients with mild to moderate liver damage need to reduce their dosage by 50%; Elderly people over 65 years old do not need to adjust their dosage, but they should strengthen the monitoring of adverse reactions.
