Orlistat is a gastrointestinal lipase inhibitor mainly used for weight management in obese and overweight patients. Its medication should pay attention to issues such as lipid soluble vitamin absorption disorders and gastrointestinal adverse reactions.

1. Indications

(1) Adult obesity treatment: It is applicable to obese patients with BMI ≥ 30kg/m ², or overweight patients with BMI ≥ 28kg/m ² and risk factors such as hypertension and diabetes. It should be combined with low calorie diet.

(2) Adolescent weight control: When the BMI of adolescents over 12 years old reaches the 95th percentile or above of their peers, it can be used under the guidance of a doctor and requires behavioral correction and dietary adjustments.

2. Medication precautions

(1) Fat soluble vitamin deficiency: Long term use may affect the absorption of vitamins A, D, E, and K. It is recommended to supplement with multivitamins daily (with a 2-hour interval before and after medication).

(2) Gastrointestinal reactions: common oily spots, urgency to defecate, etc. A high-fat diet can exacerbate symptoms. It is recommended to control fat intake within 30% of total daily calories.

(3) Special population: prohibited during pregnancy (animals showing embryotoxicity); Use with caution during lactation; Patients with chronic malabsorption syndrome or cholestasis are contraindicated.

(4) Drug interactions: Combination with cyclosporine requires a 3-hour interval; Warfarin users need to strengthen INR monitoring (reduced absorption of vitamin K may affect coagulation).

(5) Course restriction: If used continuously for more than 2 years, the efficacy and safety must be re evaluated to avoid self prolonging the course of treatment.
