The standard usage of Sorafenib is twice daily oral administration, taken on an empty stomach. For special populations, the dosage should be adjusted or monitored based on liver and kidney function, pregnancy status, etc. The following provides a detailed explanation from two aspects: conventional usage and medication for special populations, covering dose adjustment principles and contraindications.

1. Conventional usage and dosage

(1) Basic dose: 400mg each time (2 200mg tablets), taken twice a day on an empty stomach with an interval of about 12 hours (1 hour before or 2 hours after meals).

(2) Course of treatment: Continue medication until disease progression or intolerable toxicity occurs, and missed doses should not be taken.

(3) Dose adjustment: When a grade 3 adverse reaction occurs, the first dose should be reduced to 600mg per day, and the second dose should be reduced to 400mg per day; for grade 4 toxicity, permanent discontinuation of the medication is required.

2. Medication for special populations

(1) Liver dysfunction

Child Pugh A level does not require adjustment; Child Pugh B or C grade patients should use with caution, and in case of liver toxicity, the dosage should be reduced or discontinued according to the standard.

(2) Renal insufficiency

Mild to moderate renal insufficiency (creatinine clearance rate ≥ 30mL/min) does not require adjustment; Insufficient data on end-stage renal disease patients, not recommended for use.

(3) Pregnancy period

Animal experiments have shown a risk of embryo malformation, and pregnant women should avoid using it. Before taking medication, it is necessary to confirm the pregnancy status and take contraceptive measures.

(4) Lactation period

Medications may be secreted through breast milk, and medication should be stopped or breastfeeding should be terminated during lactation.

(5) Children

The safety and efficacy of using it for patients under 18 years old have not been established, and it is not recommended.

(6) Elderly people

Patients aged 65 and above do not need to adjust their dosage, but close monitoring of blood pressure and liver function is required.
