Precautions for Pazopanib and medication for special populations
Pazopanib is a multi-target tyrosine kinase inhibitor primarily used for the treatment of advanced renal cell carcinoma and soft tissue sarcoma. The precautions include monitoring liver function, cardiovascular risk, and drug interactions. Medication for special populations should be adjusted or prohibited based on pregnancy status, liver and kidney function, and other factors.
1. Precautions
(1) Abnormal liver function: During treatment, it is necessary to regularly monitor transaminase and bilirubin levels. If severe liver toxicity occurs, medication should be discontinued.
(2) Cardiovascular risk: May cause QT interval prolongation and heart failure, high-risk patients should use with caution and monitor electrocardiogram.
(3) Bleeding risk: It may increase the probability of bleeding, and patients with active bleeding or recent bleeding history should avoid using it.
(4) Hypertension: Common adverse reactions require regular monitoring of blood pressure and the use of antihypertensive drugs if necessary.
(5) Drug interactions: Avoid combination with potent CYP3A4 inhibitors or inducers as it may affect efficacy or increase toxicity.
2. Medication for special populations
(1) Pregnancy: Animal studies have shown embryotoxicity, and pregnant women should avoid using it. Women of childbearing age should take effective contraceptive measures.
(2) Breastfeeding period: Medications may be secreted into breast milk, and lactating women should stop taking medication or suspend breastfeeding.
(3) Pediatric patients: Safety and efficacy have not been established yet, and use is not recommended.
(4) Elderly patients: Patients over 65 years old should be cautious as they may be more sensitive to adverse reactions.
(5) Liver dysfunction: Patients with moderate liver dysfunction should reduce their dosage, while patients with severe liver dysfunction should avoid using it.
(6) Renal insufficiency: Mild to moderate renal impairment does not require dose adjustment, while data on patients with severe renal impairment is limited and should be used with caution.
