Lustrombopag is an oral thrombopoietin receptor agonist that increases platelet count by stimulating megakaryocyte proliferation and differentiation. It is mainly used for perioperative management of chronic liver disease associated thrombocytopenia, and its treatment scope is strictly limited to patients with specific liver function grades.
1. Function and efficacy
(1) Pharmacological mechanism: Selective activation of thrombopoietin receptor (TPO-R), mimicking the action of endogenous thrombopoietin, promoting megakaryocyte proliferation, differentiation, and platelet production.
(2) Effective characteristics: Blood drug peak concentration is reached 6-8 hours after oral administration, and platelet count usually reaches its peak 12 days after medication, which can significantly increase baseline platelet count by more than 30 × 10 ⁹/L.
(3) Continuous effect: The half-life is about 27 hours, and the efficacy can last for 5-8 days after a single administration, creating a safe time window for surgery.
2. Scope of treatment
(1) Core indication: Adult patients with chronic liver disease who plan to undergo invasive procedures (such as liver biopsy, dental surgery) for thrombocytopenia treatment need to start medication 10-13 days before surgery.
(2) Applicable conditions: Children Pugh A/B patients with baseline platelet count<50 × 10 ⁹/L and severe liver dysfunction (Child Pugh C) require careful evaluation.
(3) Prohibited population: People who are allergic to drug ingredients, patients with non liver related thrombocytopenia, and patients with uncontrolled thrombotic diseases are prohibited from using it.
