Selinexor is a selective nuclear output inhibitor primarily used for the treatment of multiple myeloma and diffuse large B-cell lymphoma. Its contraindications include allergies to drug ingredients and contraindications during pregnancy; Medication for special populations should be adjusted based on liver and kidney function, age, and fertility status.
1. Contraindications
(1) It is contraindicated for individuals who are allergic to selenidazole or any excipients.
(2) Pregnant women are prohibited due to animal studies showing embryonic toxicity and teratogenic risks.
(3) Patients with severe liver function impairment (Child Pugh C grade) are contraindicated.
(4) Patients with severe thrombocytopenia (<25 × 10 ⁹/L) or neutropenia (<0.5 × 10 ⁹/L) should suspend medication.
2. Medication for special populations
(1) Pregnancy period: The pregnancy status should be verified before medication, and effective contraception should be used during treatment and one week after discontinuation of medication.
(2) Breastfeeding period: There is no data on the distribution of drugs in breast milk. It is recommended to suspend breastfeeding within one week after discontinuing the medication.
(3) Children: Safety and efficacy have not been established yet, and it is not recommended to use it.
(4) Elderly people (≥ 65 years old): No dose adjustment is required, but adverse reactions need to be closely monitored.
