Ixazomib takes effect for a few days, medication precautions
Ixazomib, as a proteasome inhibitor, varies in efficacy depending on individual differences. It usually requires continuous medication for 2-4 cycles (28 days per cycle) to evaluate its effectiveness. When using Ixazomib, precautions must be strictly followed to ensure treatment safety and efficacy.
1. Effective time
(1) Evaluation cycle: After completing at least 2-4 treatment cycles (with an interval of 28 days between each week), the efficacy should be evaluated through bone marrow aspiration, serum M protein testing, and other methods.
(2) Individual differences: Some patients may experience symptom relief within the first cycle, but complete relief usually takes longer.
2. Medication precautions
(1) Medication standard: Take on an empty stomach on fixed days of the week (1st, 8th, and 15th), and fast for 1 hour before and after medication to ensure stable blood drug concentration.
(2) Omission treatment: If you miss the scheduled time, you cannot take the supplement or double the dose. You should skip it directly and take the medication normally at the next scheduled time.
(3) Dose adjustment: When there is ≥ grade 3 hematological toxicity (such as platelet<50 × 10 ⁹/L) or non hematological toxicity, the dosage should be gradually reduced according to the standard.
3. Adverse reaction monitoring
(1) Hematological monitoring: Complete blood cell count is checked every 2 weeks during the initial stage of treatment, and once a month in the later stage to alert for thrombocytopenia and neutropenia.
(2) Gastrointestinal reactions: common diarrhea and nausea. It is recommended to use antiemetic drugs for prevention. In severe cases, medication should be suspended and symptomatic support should be given.
4. Management of special populations
(1) Liver damage patients: moderate damage (Child Pugh B grade) reduced to 2.3mg, severe damage (Child Pugh C grade) reduced to 1.7mg.
(2) Pregnancy contraindications: It has embryo fetal toxicity, and the pregnancy status should be confirmed before medication. Patients of childbearing age should take effective contraceptive measures.
