Usage, Dosage, and Medication Guidelines for Baricitinib
Baricitinib is a JAK inhibitor mainly used to treat autoimmune diseases such as rheumatoid arthritis. Its standard dose is 2mg or 4mg orally once daily, and the dose needs to be adjusted according to the patient's condition and tolerance, while monitoring risks such as infection and thrombosis.
1. Standard usage and dosage
(1) Rheumatoid arthritis: The recommended dose for adults is 4mg orally once daily, which can be reduced to 2mg/day for individuals aged ≥ 75 years or at risk of chronic infection.
(2) COVID-19 treatment: It needs to be used in combination with remdesivir at a dose of 4mg once a day for a maximum of 14 days or until discharge.
2. Principle of dose adjustment
(1) Liver dysfunction: mild abnormalities do not require adjustment; The dose of 4mg is contraindicated for moderate to severe patients, and 2mg/day is recommended.
(2) Renal insufficiency: eGFR30-60mL/min reduced to 2mg/day; EGFR<30mL/min or prohibited for dialysis patients.
(3) Drug interactions: When used in combination with potent OATP1B3/CYP3A inhibitors, the dosage should be reduced to 2mg/day.
3. Medication precautions
(1) Infection monitoring: latent infections such as tuberculosis and hepatitis B should be screened before treatment, and bacterial/fungal/viral infections should be vigilant during treatment.
(2) Thrombosis risk: Use with caution in patients over 65 years old or with a history of thrombosis. Discontinue medication immediately if symptoms of thrombosis occur.
(3) Laboratory monitoring: Regularly check lymphocyte count, neutrophil count, and liver and kidney function.
4. Medication for special populations
(1) Pregnant women: Prohibited during pregnancy, effective contraceptive measures should be taken during medication.
(2) Breastfeeding period: It is recommended to stop taking medication or breastfeeding as medication may be secreted into breast milk.
(3) Children: The safety and efficacy of patients under 18 years old have not been established.
