The use of Lenvatinib should focus on adverse reactions such as hypertension and proteinuria, and regular monitoring and adjustment of dosage are necessary. Individualized medication plans should be developed for special populations, and attention should be paid to drug interactions and reproductive risks.

1. Serious Adverse Reaction Management

(1) Hypertension

Grade 3 or 4 hypertension may occur during treatment. Blood pressure should be controlled before medication, monitored daily during the first week of treatment, and checked regularly thereafter. If hypertension occurs, it is necessary to adjust antihypertensive drugs or suspend lenvatinib.

(2) Proteinuria

Regular monitoring of urinary protein is required during treatment. Grade 2 proteinuria requires reduction, Grade 3 medication should be suspended, and Grade 4 medication should be permanently discontinued. Patients with nephrotic syndrome are prohibited from using it.

2. Dose adjustment standard

Adjust the dosage according to weight: 24mg/day for those weighing ≥ 60kg, 20mg/day for those weighing<60kg. Suspend medication in case of grade 3 adverse reactions, and reduce dosage after recovery; Level 4 requires permanent discontinuation of medication.

3. Drug interactions

Avoid using strong CYP3A inducers in combination. Caution should be exercised when using CYP3A inhibitors in combination. Use P-gp substrate drugs with caution as it may increase their blood drug concentration.

4. Medication for special populations

(1) Liver dysfunction

Child Pugh C patients need to reduce their dosage. During the treatment period, it is necessary to strengthen liver function monitoring. If liver toxicity occurs, the dosage should be adjusted or the medication should be discontinued.

(2) Renal insufficiency

Patients with severe renal insufficiency should use it with caution. Monitor renal function during treatment, and adjust dosage if renal function deteriorates.

5. Medication operation matters

Take daily at a fixed time and can be taken with food. Missing medication does not require supplementation, please take the medication according to the original plan next time. Drug overdose requires immediate medical attention.

6. Fertility related risks

Pregnant women are prohibited from using contraception, and women of childbearing age should use contraception for 2 weeks after the last dose. Breastfeeding is prohibited during the medication period. Male patient partners also need to use synchronized contraception.
