Precautions for Cabozantinib
Cabozantinib is a multi-target tyrosine kinase inhibitor that requires strict monitoring of risks such as bleeding, thrombosis, and wound healing during use, and dosage adjustment or discontinuation based on adverse reactions. The following provides a detailed explanation of the precautions from 12 aspects.
Precautions for Cabozantinib
1. Perforation and fistula
Monitor gastrointestinal perforation or fistula symptoms, and permanently discontinue medication if present. Symptoms include severe abdominal pain, nausea, or coughing during eating, and immediate medical attention should be sought.
2. Risk of bleeding
Patients with severe bleeding (such as hemoptysis and black stool) should not use it. If bleeding of grade 3 or above occurs or surgery is required, treatment should be suspended. Avoid external injuries in daily life, pay attention to oral care and use of sharp objects.
3. Thromboembolic events
May increase the risk of arteriovenous thrombosis. Permanently discontinue medication in the event of myocardial infarction or thrombotic events requiring intervention. If there is swelling, chest pain, or difficulty breathing in the limbs, seek medical attention immediately.
4. Damage to wound healing
Medication should be stopped for at least 3 weeks before elective surgery, and resumed at least 2 weeks after major surgery with wound healing. Before dental procedures, it is also necessary to pause.
5. Hypertension management
Regular monitoring of blood pressure is required before and during treatment. Uncontrolled hypertension patients are prohibited from using the medication. If there is a crisis or the medication is difficult to control, it should be permanently discontinued.
6. Jaw necrosis
Manifested as jaw pain, gum ulcers, or delayed healing after dental surgery. Need to strengthen oral examination, and reduce dosage for recovery after occurrence.
7. Diarrhea and skin reactions
Diarrhea should be temporarily stopped until grade 1 before reducing dosage and continuing use. Palmoplantar erythema syndrome requires interruption of treatment and adjustment of dosage.
8. Hepatotoxicity and proteinuria
The risk of elevated liver enzymes increases when combined with nivolumab and requires frequent monitoring. Patients with proteinuria or nephrotic syndrome need to discontinue medication.
9. Endocrine system effects
May cause adrenal insufficiency or thyroid disorders, requiring symptomatic treatment. Hypocalcemia requires calcium supplementation and dosage adjustment.
10. Neurological risk
If epilepsy, headache, or confusion occur, reversible posterior encephalopathy syndrome should be investigated and medication should be permanently discontinued.
11. Medication for special populations
Pregnant women are prohibited from using contraception, and women of childbearing age should use contraception for 4 months after the last dose. Avoid feeding during lactation. Elderly people and those with liver and kidney dysfunction need to be carefully adjusted.
12. Omission and Overtreatment
If there is less than 12 hours before the next medication, skip if missed. There is no clear treatment plan for drug overdose.
