The dosage of Mobocetinib should be determined according to the indications and individual patient conditions. It is mainly used for non-small cell lung cancer with EGFR exon 20 insertion mutation. The standard dose is 160mg orally per day, and it needs to be continued until disease progression or intolerable toxicity occurs.

1 Standard Dose Protocol

(1) Adult non-small cell lung cancer: Take 160mg orally once a day (4 40mg capsules), it is recommended to swallow the whole capsule, do not crush or chew.

(2) Medication time: It should be taken at a fixed time every day, and can be taken with food or on an empty stomach, but the administration method should be consistent.

Principle of dose adjustment

(1) When a grade ≥ 3 adverse reaction occurs: the medication should be temporarily discontinued until it recovers to grade ≤ 1. For the first recurrence, the original dose should be continued, and for the second recurrence, it should be reduced to 120mg/day.

(2) QT interval prolongation: If QTcF>500ms, administration should be suspended until it recovers to ≤ 480ms, followed by a permanent reduction to 120mg/day.

3. Medication for special populations

(1) Liver dysfunction: Children Pugh grade C patients need to reduce their dosage to 80mg/day, and those with moderate to severe injuries need to be closely monitored.

(2) Renal insufficiency: Mild to moderate injury does not require adjustment, and there is a lack of data for end-stage renal disease patients (eGFR<15mL/min).

4 Precautions for combination therapy

(1) Strong CYP3A inhibitor: Avoid combination therapy and reduce to 80mg/day when necessary. Discontinue the inhibitor for 5 half lives before returning to the original dose.

(2) CYP3A inducer: may reduce the blood concentration of Mobotinib, alternative therapy or increased monitoring frequency should be considered.
